Complex Pediatrics Workflow

Enrollment

Patient’s provider prescribes a pathway, configured to the health system’s protocol, from within their EHR or directly within vCareCommand, enrolling the patient in the system and triggering welcome message with login credentials to be sent to the patient. For pathways that require device kits to be deployed to the patient for capture of biometrics, notification is also provided to fulfillment team to get the appropriate kit sent to the patient.

On-boarding

In-app on-boarding gets the patient familiar with the tools and capabilities of the application and secure messaging to gain access to assistance when needed

Engagement

The patient is prompted with alerts and reminders when it is time to engage with the empathetic AI chatbot, which captures patient provided data and walks patient through capture of biometrics via connected devices.

Submission  Review and Alerts

vCareCommand performs algorithmic assessment of the data captured by the AI chatbot, assigns acuity score for prioritization on the population health dashboard and triggers alerts and notifications for the appropriate care team members for urgent interventions.

Interventions

Monitoring care teams can augment care plans based on the data captured and create assignments for patients in their web portal, assign educational resources for the patient to review, secure message exchange with the patient and establish real-time virtual visits between the patient and appropriate care team members.

Documentation

Case managers document interactions and review notes in vCareCommand and the platform automatically sends periodic clinical summary notes back to the EHR to maintain a history of patient progress within the patient record for care continuity.
